You have the right to request a copy of your health care records.
To request your records, please submit a written request using one of the following methods:

Please include:

Your full name

Date of birth

The specific type of records you are requesting

How you would like to receive the records (secure email, client portal, or paper copy)

Your signature and date
Requests are processed within 15 business days, in accordance with Texas law. Reasonable fees may apply as permitted by law.
